Privacy, Data Minimization, and PHI Notice
Who we are
Access for Therapists (AFT) connects eligible students with independent licensed providers. AFT is not a healthcare provider, is not a HIPAA-covered entity or business associate, and does not provide clinical services.
What we collect
We collect the minimum necessary to verify eligibility, suggest potential matches, coordinate introductions, and evaluate pilot reach. Typical information includes your name and contact details you provide, eligibility confirmations from you or your university, and limited participation metadata such as timestamps for introductions and confirmations. Do not send medical information to AFT. We do not request, store, or transmit Protected Health Information.
How we use and share information
We use information to run AFT services, prevent misuse, and meet accountability obligations. When a university participates, limited roster confirmations may be exchanged under a signed data-sharing addendum on file with that institution. We do not sell personal information. Providers keep all clinical records in their own systems and are responsible for compliance with law and licensing-board rules.
Data handling and safeguards
We apply administrative, technical, and organizational measures appropriate to the sensitivity of the information we handle. Access is limited to staff with a need to know. We keep information only as long as needed for the pilot and applicable obligations, then remove it in line with our internal retention practices.
Provider profiles
Provider profiles are public pages showing only professional, non-PHI information; contact flows through AFT (we do not publish provider email/phone).
Your choices
You may decline to provide optional information. If you send medical information despite this notice, we will not use it to operate AFT services and will remove it to the extent feasible.
